That’s the time that Orangeburg-Wilkinson Athletes can receive a free physical from USC Sports Medicine for the 2016-2017 athletic year. If you are a current 8th, 9th, 10th, or 11th grader who plans on participating in sports next year, this is your opportunity to get your physical done for FREE!
Click here to find the South Carolina High School League (SCHSL) physical exam and parent permission forms. Remember the SCHSL requires all pre-participation exams to be completed using only this form. Please print or pick up the forms from your coach, Athletic Trainer, or Athletic Director as soon as possible to allow ample time to obtain parent/guardian signature. All forms must be completed in their entirety prior to arrival, including medical history and signatures, or no exam will be given. It is important that the Parent Permission and Acknowledgement Of Risk form is signed and dated. The exam forms should NOT be on colored paper as it makes them hard to read.
USC Sports Medicine asks that all athletes wear comfortable sports attire (ie: shorts, t-shirts, tennis shoes, etc…) that will allow the individual to perform basic movements required during the examination. It is also important to note that USC Sports Medicine will not be retaining copies of completed pre-participation exams. Physicals and Parent Permission Forms should be turned into the Athletic Director’s office at the first opportunity.
If you are a student-athlete, please let your coaches or Athletic Director know if you plan on attending this event NO LATER THAN APRIL 15TH. It is possible that we may have buses available to transport student-athletes to the event (if necessary) at a minimal cost of $10.00 per athlete.
